Once Upon a Time

This show is remarkably good at creating loose ends and then tying them up.

And then, you know, creating new loose ends.

Totally a pet peeve when a story world breaks its own rules for plot reasons (Joss Whedon, I'm looking at YOU, although it's true I kept closer track of the the Buffyverse story rules than should be legally allowed).

It throws me out of the story and makes me Stop Caring. And that's the kiss of death.

I can think of an example or two where a show somehow manages to get away with it, but in general, that can be a major source of annoyance, especially when you get the sense that the writers just don't care, or don't expect you to be smart enough to notice.
